About the Opportunity

An opportunity has become available for a Practitioner - Targeted Care Packages (TCP) to join our supportive team in Wodonga, Victoria.

Reporting to the Team Leader TCP, the successful candidate will be responsible for facilitating high-quality case management to achieve better outcomes for children/young people who are supported by a Targeted Care Package.

Key Responsibility Areas of this role will include (but are not limited to):

  • Working therapeutically with young people and their family or carers to develop and coordinate the Care Team activities and the implementation of the Case Plan
  • Taking responsibility for the implementation of the DFFH Case Contracting Agreements and managing the recording and documentation of TCP service delivery
  • Liaising with relevant services and advocating on behalf of children and young people to ensure access to resources where necessary
  • Proactively promoting and supporting a culture of child safety and relevant cultural connection for all children, including Aboriginal and Torres Strait Island children, those with a disability, and CALD

About You

To be eligible to apply for this position, you'll require a qualification in social work, welfare, community services or a related field. We're looking for someone with knowledge and demonstrated experience in delivering innovative and effective approaches for young people with complex needs up to 18 years of age. Ultimately the successful candidate will have a demonstrated ability to achieve positive outcomes for young people through a strength-based, outcomes-focussed case management approach.

All candidates will need to possess or be able to obtain prior to commencement:

  • Working with Children Check - Victoria and NSW
  • Satisfactory National Police Check
  • Current driver's licence (you will be Wodonga-based however, the role will see you travelling throughout the region from time to time

Importantly, you will be a team player with strong communication skills, highly developed analytical and problem-solving skills, and good levels of resilience. This role requires you to manage competing priorities and interact with people with various abilities and needs.
